Rodjena kao Susan Abigail Tomalin u New York-u od oca velsanina Phillip Leslie Tomalin-a i majke italijanke Lenora Marie Criscione. Pohadjala je Americki Katolicki Univerzitet 1964. gde je upoznala buduceg supruga Chris Sarandon-a od koga se razvodi posle 15 godina braka zadrzavajuci muzevljevo prezime kao svoje umetnicko.
Posle Demokratske Nacionalne konvencije 1968. otisla je na audiciju za film Joe. Iako nije dobila nijednu od glavnih uloga, vec ulogu tinejdzerke koja nestaje (film je prikazan 1970.). Nije pratila uspeh tog filma, vec je glumila u niskobudzetnijim filmovima, kao sto je Lovin' Molly, pet godina kasnije pojavljuje se u kultnom filmu The Rocky Horror Picture Show. Iste godine igra glavnu zensku ulogu u filmu The Great Waldo Pepper, pored Robert Redford-a.
Nominovana je za Oskara 1980. za glumu u filmu Atlantic City, medjutim, ne dobija Oskara. 1988. godine, za vreme snimanja filma Bull Durham, upoznaje glumca Tim Robbins-a, s kim je u vezi i dan danas.
U devedesetim biva nominovana za Oskara u tri filma (The Client, Lorenzo's Oil, Thelma and Louise), medjutim tek 1996. dobija Oskara za ulogu u filmu Dead Man Walking,
